Terms and AgreementsAvacen Rental Terms(Required)2.1 Rental Fee & Payment The rental fee is $200.00 per week, due in full at the time of pickup. The rental period is seven (7) calendar days. Any extension must be arranged in advance and is subject to availability and additional fees. 2.2 Replacement Fee & Liability The Renter acknowledges that the AVACEN 100 device has a replacement value of $3,000.00. The Renter assumes full financial responsibility for any loss, theft, damage beyond normal wear and tear, or failure to return the device. In such event, the Renter agrees to pay the full $3,000.00 replacement fee. I agree to the rental termsREQUIRED CONDITIONS OF USE(Required)The Renter agrees to comply with ALL of the following conditions. Failure to do so may result in immediate termination of the rental agreement, forfeiture of the rental fee, and/or liability for damages. 3.1 Protective Glove — REQUIRED EVERY USE The Renter must wear the provided protective glove/mitt EVERY TIME the device is used without exception. Bare-hand use is strictly prohibited and may result in injury and immediate termination of the rental. 3.2 Instructions for Use The Renter agrees to follow all manufacturer instructions, as reviewed at pickup: Read and follow the complete AVACEN 100 User Manual provided with the device. Use the device only as directed — place the hand inside the vacuum chamber with the protective mitt on. Do not exceed the recommended session duration. Do not attempt to repair, modify, or disassemble the device. Keep the device away from water and liquids. Store the device in a safe, climate-appropriate location when not in use. Do not allow children to operate the device unsupervised. 3.3 Authorized Users The device is for the use of the Renter and members of their immediate household residing at the address listed above. The Renter may not sublease, lend, or transfer the device to any third party. 3.4 Medical Disclaimer The AVACEN 100 is an FDA-cleared wellness device for temporary relief of minor pain. It is not intended to diagnose, treat, cure, or prevent any disease. The Renter is advised to consult a healthcare provider before use, especially if pregnant, have a pacemaker, active implants, or circulatory conditions.
